Memberships Contract

Our membership programs are designed to help our clients wishing to take their beauty and skincare maintenance to the next level by providing regular monthly treatments at a significant discount. Members enrolled in the Skincare Membership are entitled to receive one treatment every month. You may choose from a Signature Facial, a Hydrodermabrasion Facial, a Dermaplaning Facial, or a Chemical Peel. Members enrolled in the Eyes to Mesmerize Membership are entitled to receive one Lash Lift/Tint treatment every 6 weeks. You may choose to enroll in one membership or both the Skincare and Eyes memberships and will be charged for each enrollment separately.

Additional membership benefits include a 15% discount on all retail products.

The Initial Membership period shall be for a minimum period of 6 months (The Initial Period) and includes a one-time setup fee of $25. A member shall not be entitled to cancel his/her membership during the Initial Period. After the Initial Period, the membership shall continue month to month. After the Initial Period, the member may cancel the membership at any time by giving a 30-day written notice. Any unused services will expire should you elect to cancel your membership.

Unused services can be carried over to the next month. A maximum of 3 services can be accumulated in this manner.
Additional unused services will be forfeited.

Your membership services are not transferable. They must be used by the member enrolled in the program.

Appointments are made on a first-come, first-served basis. You are responsible for scheduling your monthly services
in advance to ensure you receive your preferred date and time. If you are unable to keep a scheduled appointment, you must cancel at least 24 hours prior to your appointment or that service will be forfeited.

Membership dues do not include gratuities. Membership services may not be combined with any other
promotional offer.

Portland Birth and Body LLC reserves the right to cancel any membership at any time.

A valid credit card is required to enroll in any of the Portland Birth and Body LLC Membership programs. You agree to provide us with updated information if your address or payment information should change. If we are unable to process the automatic monthly membership dues, the membership will be suspended pending payment in full. Any accumulated services will be forfeited if the membership is suspended due to nonpayment.

By signing up for the subscription I hereby agree to the Membership Contract as stated above, I authorize Portland Birth and Body LLC to charge the credit card on file according to the terms of this agreement. Membership fees will be charged to the designated credit card each month from the day of enrollment for the Skincare Membership and/or every six (6) weeks from the time of enrollment for the Eyes to Mesmerize Membership.

By subscribing online, I hereby authorize Portland Birth and Body LLC to charge my credit card according to the terms of this
membership agreement.
